Belt Testing Form

Total Impact Martial Arts requires all students to conduct themselves in a positive manner and uphold the principles of our student creed. Students are expected to treat others with respect and courtesy and always be accountable and responsible for their own actions. For students to be eligible for belt rank they must have the knowledge and understanding of the required material but also uphold the principles of our student creed, mission statement and school policies. These teachings will always guide students to conduct themselves with honor, respect and hard-work.

We expect our students to be positive leaders, role models and examples of Black Belt excellence within the community. Before signing, please review these questions:

Students must have this form completed and signed by parents and teachers to test. All students must have all the required stripes and be approved to test by an instructor. All fees must be turned in prior to belt testing!
